PP&D Post Activity Evaluation Form


Please complete this report no later than 1 week after your sponsored community activity.  Include all information below and attach additional documentation if necessary. 


A copy of this report will be submitted to the Program Planning & Development email address (ppd@bcacdeltas.org) upon submission.  Attach programs, news articles, press releases, and any other pertinent information along with this post-activity report.  A copy of your evaluation form summary should also be attached.

If your committee conducts an activity that occurs over the course of several months and is the same activity repeated at each occurrence (i.e., afterschool tutoring), you are only required to submit one Post-Activity Report no later than 1 week after the conclusion of the repeated serial activity.

If your committee sponsors a series of activities each with a different purpose (i.e., a series of financial workshops each with a different topic), you are required to submit a Post-Activity Report no later than 1 week after the conclusion of each activity.


Chapter meetings are being held virtually at this time.  Meeting details are emailed to members prior to each chapter meeting.
